The Vial of LIFE (Lifesaving Information for Emergencies) is an easy way for seniors or anyone to have important medical information on hand just in case of an emergency. You can have one in your home and/or car and it does not take up much room.
Instructions included below are in English and Spanish.
The information you should include would be a recent picture, medications, allergies and recent illnesses as well as your emergency contact person. Even what language you speak.

Having this information available to Emergency personnel could save valuable time.
There are just a few simple steps to get your Cobb Senior Citizen Vial of LIFE ready:
  1. Fill out the Vial of Life form. Put the form in the Vial of Life baggie with any other medical information you feel could help in an emergency including a picture of yourself for Emergency Personnel. Also note any recent or chronic illness and doctor's name and phone number.

  2. Included in the package is a baggie and large sticker which should be put on the baggie.

  3. Securely tape or use a magnet to put the plastic baggie on the front of your refrigerator door.

  4. We recommend that the form be updated every 6 months perhaps when you change smoke detector batteries, or celebrate a birthday or anniversary, etc.
Vial of LIFE kits are available at all Cobb County Senior Centers.
